To ensure the safety and well-being of every camper, our summer camp requires all physician-signed health forms to be submitted digitally before your child’s first day. These forms must be signed by a licensed physician and uploaded to our registration portal as a clear, legible PDF or image file. Please make sure all fields are completed and the physician’s signature is visible before submitting.

For Android users, Google Drive app is a quick and effective way to scan documents.
For iOS users, you can use Apple Notes
Scanning with a scanner
Scanning with an actual scanner is old school, but still the best way to get a high quality digitized result. These days there are comparatively low cost small and portable scanner devices on the market, for example by Brother.
Fax your documents
If you are unable to upload or email your forms, you may also fax your documents to (313) 484-0804. Sometimes this is the only option, depending on your doctor’s office.
Upload your scanned documents
Don’t forget to upload your completed form files and medical documents to our Document Center.
*NOTE: we do not accept cell phone or camera photos of these documents, and you will be asked to resubmit.
